We accept cash, checks, and all major credit/debit cards. We also accept CareCredit. We ask that you pay your estimated out-of-pocket cost at the time of service (estimate amounts based on expected insurance so are not guaranteed). Call us for more details!
Many small insurance providers use the doctor networks of the companies listed above. You can check with our office or your insurance company to verify our 'in-network' status. We submit insurance claims as a courtesy and can't be responsible for denial of payment. Any amounts not covered by insurance are the patient's responsibility.
Our team at Birdwell & Mutlak Dentistry respectfully requests a notice of two business days to change or cancel an appointment. This allows us to make that time available to other patients that may have immediate needs. We reserve the right to charge a $40 fee to offset the cost of unused staff time if an appointment is missed or cancelled without sufficient notice or reason (e.g. illness). Appointment reminders like emails, texts, and calls are a courtesy. Please note your appointment in your calendar or smartphone as soon as you make it.
The privacy of your health information is very important to us. We are required by federal and state law to maintain the privacy of your health information. We are also required to give you details of our privacy practices and your rights concerning your health information. Our Notice of Privacy Practices, effective April 14, 2005, outlines the privacy practices of this office. If we make a significant change in our privacy practices, we will make a new notice available to you.
