Many small insurance providers use the doctor networks of the companies listed above. You can check with our office or your insurance company to verify our "in-network" status.  

We submit insurance claims as a courtesy and can't be responsible for denial of payment. Any amounts not covered by insurance are the patient's responsibility.

Birdwell & Mutlak Dentistry accepts cash, checks, and all major credit / debit cards. We also accept CareCredit, a healthcare credit card that can allow you to make payments without interest. We ask that you pay your estimated out-of-pocket cost at the time of service (estimate amounts based on expected insurance so are not guaranteed).
